We know that individuals with untreated mental illness and/or substance use disorders often find themselves in emergency departments, inpatient hospitalization, and local jails. These interventions are costly and frequently do not yield the desired outcomes. It is important for communities to identify treatment gaps and develop strategies to improve continuity of care.
