ARC: Planning for the Future Series - Transitioning to Adult Health Care

The Arc of San Antonio hosts workshops and resource fairs designed to help people with intellectual and developmental disabilities (IDD) and their families reach their goals and dreams by planning for a future that allows them to live as independently as possible. Each session will start with a 30-minute meet and mingle over coffee. The workshops will begin at 9:30 a.m. followed by an hour-long resource fair applicable to each session.
Transitioning to Adult Health Care: An important step in the future planning process, this session will discuss transitioning the Medical Home, Dental and Vision, Behavioral Health, Therapies, and Diagnostics.
