2005 LISTING OF SERVICES
The following is a list of HIV/AIDS services available in the San Antonio Eligible Metropolitan Area. Although we have done our best to provide accurate information, changes occur frequently. Each service provider should be contacted to verify availability of the service and eligibility requirements.
Ambulatory/Outpatient Medical Care Ambulatory/Outpatient Medical Care includes the provision of comprehensive office-based medical services to include treatment education and prophylactic treatment for opportunistic infections. Focuses on the management of chronic and acute conditions and illnesses and prevention of illness. All medical services adhere to established HIV clinical practices and Public Health Service Guidelines.

Case Management Case Management is a working agreement between a client and a case manager for a defined period based on the client's assessed needs. The purpose of case management is to assist clients with HIV/AIDS to get into care and/or remain in care by the coordination and/or procurement of needed services so that the problems associated with living with the disease are mitigated.

Client Advocacy Client Advocacy provides advice and assistance to qualified individuals to obtain medical, social, community, legal, financial, & other needed services. Advocacy does not involve assessment, coordination and follow-up on medical treatments, as case management does.

Direct Housing Assistance
Housing assistance provides emergency short-term rental, transitional, or mortgage help intended to keep individuals from becoming homeless and to enable them to gain and/or maintain medical care. Use of Ryan White Care Act funds for short-term or emergency housing must be linked to medical and/or healthcare or be essential to a clients ability to gain or maintain access to HIV-related medical care or treatment.

Drug Reimbursement (Local and State) Local funds will pay for approved pharmaceuticals for individuals with no other payment source. May also be used to augment approved state-operated pharmaceuticals payment/reimbursement programs (AIDS Drug Assistance Program – ADAP, Medication Plus, etc.). Medication must be purchased at the lowest possible cost, preferably public health service guidelines or 340 B pricing.

Emergency Financial Assistance Emergency financial assistance helps provide short-term payments for essential utilities, medication assistance—when other resources are not available. The goal of the Emergency Financial Assistance program is to assure clients' essential utilities or medications are maintained to ensure continued access and maintenance of HIV-related medical care.

Family Support Services Support Services for families' with children infected/affected by HIV/AIDS to include transportation, foster care, provision of day care and/or early childhood education services, respite services, and related adoption/legal service as needed, if not available in the community.

Food Bank / Home Delivered Meals Provision of food or meals. Provision of actual food (not finances to purchase food) and related grocery items, to include fresh fruit, vegetable, and staples in a food pantry setting in a cost effective way. Does not include nutritional education or counseling which is provided under the service category of Nutritional Counseling. The Home Delivered Meals programs provides hot and cold or frozen nutritionally balanced and home delivered meals for eligible individuals who are homebound and cannot stop for or prepare their own food. Food vouchers, such as grocery store gift cards, can only be distributed to clients in combination with a food bank visit to purchase a specialized food item or in an emergency.

HIV Health Education / Risk Reduction Provide HIV/AIDS care and treatment information designed to enable clients to make informed decisions about their HIV medical services options and information to effectively access appropriate services. The goal of this service is to ensure improved health and HIV/AIDS primary medical care outcomes by addressing and responding to the individual needs of people living with HIV through the creation and broadly dissemination of information relevant to the prevention and treatment of HIV, new research gains and ongoing studies, emerging trends in the epidemic, and common-sense strategies for self-advocacy. It is preferred that these services be provided by HIV positive educators.

Health Insurance Continuation A program of financial assistance for eligible individuals living with HIV to maintain a continuity of health insurance or to receive medical benefits under a health insurance program. The goal of the Health Insurance Program is to provide cost-effective ongoing access to and continuity of primary medical care of a client's HIV-disease. Allowable costs include premium payments, risk pools, co-payments, and deductibles.

Home Health Care Home health care services are therapeutic, nursing, supportive and/or compensatory health services provided by a licensed home health agency in a home/residential setting. Services are provided in accordance with a written, individualized plan of care established by a case management team that includes appropriate health care professionals.

Hospice Hospice care services include nursing, counseling, bereavement, supportive and/or compensatory palliative health services by a licensed agency for patients in their terminal stages of the illness. Services will be provided in a home/residential setting in accordance with a written individualized plan of care established by an interdisciplinary team that includes appropriate health care professionals and volunteers.

Mental Health/Psychiatry Psychological and psychiatric treatment and counseling services to individuals with a diagnosed mental illness, conducted in a group or individual setting, and provided by a mental health professional licensed or authorized within the State to render such services. This typically includes psychiatrists, psychologists, and licensed clinical social workers. The goal of this service is to ensure improved health and HIV/AIDS medical care outcomes by addressing and responding to the individual needs of people living with HIV/AIDS.

Nutritional Counseling The provision of nutritional education and/or counseling services provided by a licensed/registered dietician outside of a primary care visit.

Oral Health Care / Dental Care This program will provide routine and emergency dental care for persons with HIV/AIDS. To include diagnostic, prophylactic, and therapeutic dental services and the equipment necessary to adequately treat and maintain good oral health. Cosmetic dentistry for cosmetic purposes only is prohibited.

Psychosocial Support services Psychosocial Support Services consist of services, which support traditional medical approaches by enhancing quality of life, improving stress response, and promoting healthy lifestyle behaviors. These services must be referred though the primary care provider. The goal of this service is to ensure improved health and HIV/AIDS medical care outcomes by addressing and responding to the individual needs of people living with HIV/AIDS.

Skilled Nursing/Housing The provision of short-term assistance to support temporary or transitional housing (from hospital to independent living) that provides 24 hour skilled nursing care to enable an individual or family to gain or maintain medical care. Related housing may be housing in medical treatment programs for chronically ill clients (e.g. assisted living facilities), specialized short term housing, and transitional housing for clients who are HIV infected and in need of 24-hour skilled nursing care. A strategy to identify, relocate, and ensure the individual and/or family is moved to or capable of maintaining a long term/stable living situation must be developed and evaluated in a treatment plan every 90 days. The facility must be licensed by the Texas Department of Health to provide 24-hour skilled nursing care.

Substance Abuse Provision of treatment, prevention and/or counseling services to address substance abuse problems, including alcohol, legal and illegal drugs, in an outpatient or residential health service setting. Non-infected individuals who are in the HIV infected person's support system may also receive this assistance, as long as the services have at least indirect benefits to a person with HIV infection. Programs provide systemic team approaches with providers licensed within the laws of the State of Texas (Reference the Texas Commission on Alcohol and Drug Abuse, Counselor Licensure, Chapter 150, Definition "Qualified Credentialed Counselor"). The goal of this service is to ensure improved health and HIV/AIDS medical care outcomes by addressing and responding to the individual needs of people living with HIV/AIDS.

Targeted Outreach Services Provides for services to identify people with HIV disease, particularly those who were diagnosed in a hospital and/or emergency room, so that they may become aware of; and may be enrolled in ongoing HIV primary care and treatment. Outreach activities must be planned and delivered in coordination with federal, state and local prevention and HIV/AIDS care and treatment services to avoid duplication of effort and to coordinate targeted outreach services across provider organizations. The outreach provider must demonstrate knowledge of and acceptance by the targeted outreach subpopulation.

Transportation Services Transportation services helps clients to access health care or other support services in the coverage area. Types of transportation include the Agency Conveyance System, transportation vouchers (bus passes), and in emergency situations only, taxi services. Transportation services are for clients that have no other means to keep care appointments. The Agency Conveyance System is a service which provides transportation to and from medical or social service appointments in an agency owned and operated vehicle.
